Becoming a Provider
Thank you for your interest in providing services to our members. New Avenues offers a variety of products in the marketplace to meet the needs of employer groups, Third Party Administrators (TPA) and Health Maintenance Organizations (HMO). Each product reflects a different provider network.
Network Descriptions:
New Avenues Employee Assistance Program (EAP) - Contracted through specific employers.
New Avenues Behavioral Health (Gatekeeping) - Contracted through specific employers in addition to EAP benefits, providing behavioral health network and oversight of authorizations.
Midwest Behavioral Health Network HMO - Specialty behavioral health management network.
Midwest Behavioral Health Network PPO - Specialty behavioral health management network.
Advantage Preferred Plus - Medicare PPO - For Advantage Preferred Plus members.
How to Apply:
New Avenues, Inc. is pleased to be a participating health plan with The Council for Affordable Quality Healthcare (CAQH). The Universal Credentialing DataSource, within CAQH, is a web-based solution that enables healthcare providers to complete their credentialing application online.
Please download, complete and fax to New Avenues at 574-271-5980 New Avenues Provider Inquiry Response Part I - Request for Application (see link below) together with a current copy of your resume. Upon receipt of these documents New Avenues will work with CAQH to determine whether or not you have already completed a CAQH application.
If you have already completed a CAQH online provider application, New Avenues will download your provider application directly from CAQH.
If you have not completed a CAQH online provider application, upon receipt and review of your Inquiry Response Part I, New Avenues will add you to our CAQH roster, at that point you will receive a registration packet from CAQH issuing you a provider ID number. With this ID number you may log into the CAQH system at https://upd.caqh.org/OAS/ to complete your provider application online. Upon completing the application process, CAQH will notify New Avenues that your application is complete and available for us to download.
Important Steps to Take:
1. Please take a moment to make sure that your CAQH provider application is current, not expired.
2. That New Avenues is authorized in CAQH to download your CAQH application.
